About the Role
Provide personalized patient care in the home setting, utilizing the nursing process to deliver professional nursing services.
Responsibilities
- Utilize the nursing process—assessment, diagnosis, planning, implementation, and evaluation—to deliver professional nursing services.
- Document human responses to health issues and collaborate with patients, families, and healthcare colleagues to plan and evaluate care.
- Perform in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up.
- Implement care plans, coordinate continuum of care, and provide patient education and triage.
- Operate within the scope and standards of nursing practice, adhering to state regulations and professional ethical guidelines.
Requirements
- Graduate from an accredited nursing program; Bachelor's degree preferred.
- Minimum of one year of professional nursing experience preferred.
- Current unencumbered RN license; multistate licensure if applicable.
- Valid driver's license and Basic Life Support (BLS) certification required.
- Obtaining and maintaining department-specific competencies and certifications is essential; OASIS Certification (COS-C) preferred.
