About the role

The RN works under the supervision of the Nurse manager and the Director, responsible for assessing, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis. The role involves working in various settings within Riverside Health System.

Responsibilities

  • Affirms standards of practice as it relates to the specialty or patient population served (e.g., med surg, hospice, IV therapy).
  • Develops initial and ongoing comprehensive assessments of clients' needs, including OASIS assessments at appropriate points.
  • Supervises and delegates non-professional staff to ensure optimal patient care.
  • Recognizes and responds to emergency situations.
  • Evaluates own needs for support and uses identified systems and resources to meet patient care needs.
  • Supervises LPN/LVN's, and Home Health Aides in providing services.
  • Completes required clinical documentation consistently per RHS standards, including completing and "locking" patient notes and verbal orders, as well as completing certifications and recertifications after departmental meetings.
  • Completes required documentation no later than the last day of the month for timely billing.
  • Implements/initiates a plan of care and revises when necessary, providing services and/or treatments requiring specialized nursing skill, counseling clients and families in meeting nursing needs, coordinating services, informing physicians and other staff of changes in client needs, and planning for patient discharge from services.
  • Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures contributing to the physiological and psychosocial outcomes and stability of the patient.
  • Participates in case conferences.
  • Provides education to clients and families.
  • Participates in training and support of all team members, volunteers, and students as needed.

Requirements

  • Registered Nurse (RN)
  • Bachelor's Degree in Nursing (Preferred)
  • Staff Nurse (Preferred)
  • Home Care (Preferred)

Qualifications

  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 days (Required)
  • Driver's License (Required)

Actual pay is determined based on job-related factors such as relevant experience, education, credentials, skills, internal equity, and business needs.

Pay

Hiring Range: $85,800.00 - $117,975.00/Annual
