RN - Home Care - Weekends (Non-Exempt)

Job Summary Provides comprehensive home health nursing care, including case management, patient assessments, post-hospitalization follow-up, recertifications, discharges, and patient education. Performs skilled nursing duties such as dressing changes, IV therapy, access site care, and general patient care according to established policies. Assists with home safety and community program education, and ensures documentation using the Epic system. Position Requirements Graduation from an accredited nursing program and valid RN licensure.Minimum of three years of nursing experience, with at least one year in a hospital or home care setting.Valid driver’s license, auto insurance, and ability to drive long distances and in adverse conditions.Physical ability to lift up to 50 pounds, transfer patients, and perform mobility tasks.Strong communication skills, ability to follow complex instructions, and proficiency with computer systems.CPR certification biannually and knowledge of CMS Home Care regulations. Preferred qualifications include basic computer skills, wound vac application, and telehealth monitoring.
