RN Home Care Intake Coordinator
Location: On-site at a major hospital in Livingston, NJ
About the role
As a Home Care Intake Coordinator, you’ll be the vital link between hospital care and home health services. Based on-site at a leading hospital in Livingston, you’ll assess patients’ needs, educate families, and coordinate seamless transitions to home care and hospice—ensuring continuity, compassion, and quality every step of the way.
- Participate in a comprehensive two-week clinical orientation covering EMR systems, safety protocols, and compassionate care practices.
- Assess patients with complex medical conditions and develop tailored care plans for home health or hospice services.
- Educate patients and families on care strategies, resources, and expectations for home-based recovery.
- Coordinate timely and appropriate referrals to VNA home care services, ensuring smooth transitions and continuity of care.
- Collaborate with VNA clinicians and external providers to optimize patient outcomes.
- Document changes in patient condition and update care plans in real time.
Requirements
- Current RN license in New Jersey and graduation from an accredited nursing program.
- Minimum of 2 years of experience in home health nursing or clinical care coordination.
- Strong communication skills to educate, advocate, and collaborate effectively.
- Ability to prioritize and stay organized in a fast-paced hospital setting.
- A compassionate, patient-first mindset with sound clinical judgment and problem-solving skills.
Working Conditions
Fast-paced healthcare environment with phone, scanner, email, fine motor hand movement, and computer use. Use of personal automobile with valid insurance and driver’s license.

Pay
The compensation range for this position is USD $43.00 to USD $51.00 per hour.
