RN Home Care Infusion- Per Diem (North NJ)

Responsibilities
- Provide professional nursing care to patients in their home through assessment, development, implementation, and evaluation of home nursing care needs on an individual and continuous basis.
- Demonstrate competency with infusion therapy.
- Ensure compliance with physician orders in a timely manner, including visit frequency.
- Document care to reflect compliance with physician’s orders and progression toward goals.
- Follow policy, procedure, and standards of care for assessment, planning, intervention, and evaluation of patient care.
- Provide holistic care for patients and their significant others.
- Ensure safe patient care and demonstrate knowledge of assigned treatments and procedures.
- Utilize an interdisciplinary team and document all activities accurately.
- Make clinical judgments and critical thinking decisions reflecting professional standards and codes of ethics.
- Document clinical decisions appropriately in the EMR system.
- Identify appropriate payors and comply with authorizations for self and other disciplines.
- Maintain visit frequency within authorization range and use secure messaging for insurance communications.
- Complete nursing assessments, medication reconciliation, and collaborate with physicians on the Plan of Care.
- Update care plans within two weeks of changes in patient status.
- Implement teaching strategies consistent with goals and document them appropriately.
- Report patient care problems to management or physicians with possible solutions.
- Act as a liaison among physicians, managers, and healthcare team members to coordinate services.
- Communicate patient information and Plan of Care during transfers or time-off.
- Complete routine assignments accurately, consistently, and independently in a timely manner.
- Meet productivity requirements consistently.
- Maintain awareness of updated standards and incorporate them into practice.
- Demonstrate flexibility with job responsibilities and adhere to organizational competencies.
- Lift a minimum of 50 lbs., push and pull a minimum of 50 lbs., and stand a minimum of 8 hours a day.
Requirements
- Associate's Degree in Nursing or Diploma Certificate in Nursing.
- Minimum of one (1) or more years of RN experience.
- Ability/experience with inserting IVs.
- Competency requirements:
- With previous and recent IV experience (preferred) within one (1) year: Demonstration with preceptor inserting peripheral line, accessing ports, PICCs, and central lines.
- Without previous experience or experience not within one (1) year: Complete a vascular access device (VAD) course through HMH and follow-up practicum in an outpatient infusion setting, including return demonstration with preceptor.
- Successful completion of Infusion Test within probationary period with a grade of 80% or higher.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Google Suite and/or Microsoft Office platforms.
Preferred Qualifications
- Bachelor of Science in Nursing Degree (BSN).
Licenses and Certifications
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification.
Pay
Minimum rate of $45.27 hourly. The starting rate of pay is provided for informational purposes only and may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget considerations. Full-time and part-time (20+ hours/week) positions include a comprehensive benefits package, including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits.
