RN Home Care Infusion- Per Diem (Monmouth/Ocean/Middlesex counties)

Responsibilities

• Provide professional nursing care to patients in their home.
• Assure compliance with physician orders and visit frequency.
• Document care needs and progress effectively.
• Care for patients and significant others holistically.
• Ensure safe patient care.
• Complete assigned treatments and procedures safely.
• Collaborate with interdisciplinary teams and document activities.
• Identify appropriate payors and remain compliant with authorizations.
• Meet payor source guidelines and maintain visit frequency.
• Utilize secure messaging for authorizations.
• Update care plans and teach patients as needed.
• Report and communicate patient care problems promptly.
• Coordinate services effectively and meet client needs.
• Communicate patient information and care plans during transfers.
• Meet productivity requirements consistently.
• Stay updated on current practices and guidelines.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of one (1) or more years of RN Experience.
• Ability/Experience with Inserting IVs.
• Competency Requirements: Demonstrate IV insertion skills or complete a VAD course and practicum.
• AHA Basic Health Care Life Support HCP Certification.
• NJ State Professional Registered Nurse License.
