About the Role

Provide patient-centered care in clients' homes, collaborating with healthcare professionals to develop and implement care plans, supervise home health aides and nurses, and ensure quality documentation and assessments.

Responsibilities

• Develop, revise, and implement individualized care plans in partnership with physicians and team members
• Supervise care delivery by aides and nurses, providing instruction and completing supervisory visits
• Perform comprehensive assessments, accurate documentation, and medication reconciliation
• Ensure patients meet eligibility criteria
• Communicate effectively with staff, patients, and families to promote quality care
• Participate in team activities, orientation, performance improvement, and survey readiness
• Respond to diverse home situations, demonstrating flexibility and cultural competence

Requirements

• Current and unrestricted RN licensure
• Valid driver's license, reliable transportation or access to public transit

Qualifications & Skills

• Preferred: CPR certification
• Preferred: 1+ years of Home Health experience
• Strong communication and organizational skills
• Ability to work independently
