Job Description & Requirements RN Discharge Planner StartDate: 10/26/2026 Pay Rate: $1800.00 - $2500.00 POSITION SUMMARY – RN Discharge Planner POSITION DUTIES – Performs case management and coordinates the discharge planning care process of age-appropriate in-patients with chronic diseases and special health care needs within Multi-Service Unit, Intensive Care Unit, OB Units and Adolescent Care Unit by maintaining a tracking system for referred patients for admission with other healthcare facilities by performing chart reviews for requested services for return to facility or discharge home. Minimum Required Qualifications – Valid and unrestricted Registered Nurse license in any U.S. State or Territory. Valid American Heart Association (AHA) certification in Basic Life Support (BLS) and must maintain certification.2+ years experience in discharge planning and Med-Surg experience. RN experience in a clinical, medical/surgical, acute and/or critical care setting. LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – Monday to Friday, 8:00 a.m. to 5:00 p.m. with one-hour lunch (8-Hour Shift) SYSTEMS – RPMS START DATE – ASAP Facility Location Located just north of Window Rock, Fort Defiance is one of Apache County's most charming towns.
