RN Coordinator PRN
Duties
- Performs all transfer center functions, including centralized patient placement, in a timely, customer centered professional manner with the aid of a computer database.
- Captures and maintains accurate data in both patient records and center workflow systems.
- Coordinates patient transfers from various referral sources to final destination facilities.
- Facilitates patient throughput to a final destination facility, focusing on quality and efficiency.
- Follows up with all participants of the transfer process.
- Works collaboratively with others and promotes teamwork.
- Assists team members with heavier workloads.
- Serves as a consultant and mediator for physicians, nurses, and ancillary staff.
- Communicates with referring facilities and receiving facilities regarding patient transfers.
- Uses problem-solving skills to determine the best patient clinical outcome.
- Tracks and trends barriers to patient intake flow and develops action plans to improve processes.
- Attends daily bed management meetings and communicates resources and issues to appropriate areas.
- Adheres to regulatory requirements and complies with established policies and procedures.
- Escalates issues through the established chain of command and recommends process changes if needed.
Qualifications
- Graduate of an accredited nursing program.
- Bachelor’s degree in Nursing preferred.
- Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
- RN License in the states where the HCA facilities are located.
