RN Coordinator PRN

Duties

• Performs all transfer center functions, including centralized patient placement, in a timely, customer centered professional manner with the aid of a computer database.
• Captures and maintains accurate data in both patient records and center workflow systems.
• Coordinates patient transfers from various referral sources to final destination facilities.
• Facilitates patient throughput to a final destination facility, focusing on quality and efficiency.
• Follows up with all participants of the transfer process.
• Works collaboratively with others and promotes teamwork.
• Assists team members with heavier workloads.
• Serves as a consultant and mediator for physicians, nurses, and ancillary staff.
• Communicates with referring facilities and receiving facilities regarding patient transfers.
• Uses problem-solving skills to determine the best patient clinical outcome.
• Tracks and trends barriers to patient intake flow and develops action plans to improve processes.
• Attends daily bed management meetings and communicates resources and issues to appropriate areas.
• Adheres to regulatory requirements and complies with established policies and procedures.
• Escalates issues through the established chain of command and recommends process changes if needed.

Qualifications

• Graduate of an accredited nursing program.
• Bachelor’s degree in Nursing preferred.
• Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
• RN License in the states where the HCA facilities are located.
