Care Coordinator RN PRN

Role Overview: The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates care across the continuum, and engages with patients and families to ensure care goals are met. They manage discharge planning, facilitate communication among care teams, and address barriers to timely discharge. Key Responsibilities: Conduct psychosocial and functional assessments, collaborate with physicians and interdisciplinary teams, develop individualized discharge plans, and facilitate post-acute referrals. They monitor care progression, identify delays, and work proactively to resolve obstacles, ensuring efficient patient transitions. Documentation of assessments, care plans, and interactions is essential. Qualifications & Skills: Requires an RN license (single or multi-state), BLS certification, and at least 1 year of acute care nursing experience. Must possess excellent communication, organizational, and problem-solving skills, with knowledge of healthcare regulations and community resources. Ability to work collaboratively and handle stressful environments is vital.
