RN, Care Coordinator - PRN

Saint Luke’s East Hospital in Lee’s Summit, MO is seeking a Nurse Care Coordinator to join our team.

About the role

The Care Coordinator is responsible for the big-picture focus of care transitions through the continuum. An integral part of the multi-disciplinary team, the role ensures quality patient outcomes, monitors resource utilization, and mitigates obstacles to care delivery through interdisciplinary collaboration, communication, and coordination. This includes developing and carrying out discharge plans and addressing barriers that impede care progression.

Responsibilities

• Assess patient needs and develop individualized discharge plans.
• Identify additional services needed and make appropriate referrals.
• Lead and facilitate multi-disciplinary rounds.
• Ensure clinical readiness for discharge.
• Collaborate with the multidisciplinary team.
• Assist with throughput management and mitigate delays in care.
• In collaboration with the attending physician, identify appropriate length of stay and monitor resource utilization.
• Work with patients, families, and caregivers to ensure patient needs are met and a timely, appropriate discharge plan is implemented.
• Maintain appropriate documentation.

Requirements

• Registered Nurse (RN) licensure.
• 3–5 years of applicable experience.
• Basic Life Support certification (American Heart Association or Red Cross).
• Bachelor’s Degree in Nursing (BSN required within 3 years of start date; not required in Critical Access Hospitals).

Schedule

PRN, Day shift. Requires availability for 3 shifts within a 6-week period, including weekends and holidays as needed.
