Duties
- Performs all transfer center functions, including centralized patient placement, in a timely, customer centered professional manner with the aid of a computer database.
- Captures and maintains accurate data in both patient records and center workflow systems.
- Coordinates transfer of emergent, urgent, and elective patients to a final destination facility from various referral sources.
- Facilitates patient throughput to a final destination facility with a focus on quality and efficiency.
- Follows up with all participants of the transfer process.
- Works well with others and promotes teamwork by demonstrating good team member behaviors.
- Assists team members with heavier workloads.
- Serves as an example of appropriate staff behavior when on or off calls.
- Ensures accurate and concurrent data collection and maintenance in both patient record and center workflow systems.
- Uses problem-solving skills to determine the best patient clinical outcome, the physicians' preferences, and the transferring and receiving hospitals.
- Tracks and trends barriers to patient intake flow; makes recommendations and develops action plans to improve processes and systems.
- Participates in daily bed management meetings when working in the CPP function of the transfer center (possibly virtual).
- Communicates resources and issues to appropriate areas within each facility.
Qualifications
- Graduate of an accredited nursing program required; Bachelor’s degree in Nursing preferred.
- Minimum of 3 years’ related clinical experience in an acute care setting (ED or Critical Care preferred).
- Must obtain knowledge of state and national rules and regulations, including EMTALA.
- Must adhere to established policy and procedure and standards of care; escalate issues through the established chain of command in a timely manner.
- Receives recommendations for process change if needed to create a more efficient customer-centered process.
- Attends staff meetings, completes annual educational requirements, and other specified learning activities on time.
- Utilizes other team members and their experience and background when applicable to ensure the best plan of care for the patient.
