About the role
Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. They participate in medical, administrative, research, and teaching duties, and are an active member of the multi-disciplinary transplant team.
Responsibilities
- Development and implementation of IPD critical pathways
- IPD discharge planning
- Facilitating and managing the use of home care
- Triage of OPD and IPD medical issues with referral to appropriate health care professionals
- Communications with insurance companies and referring physicians
- Quality assurance projects
- Promoting health maintenance education and follow-up
- Donor & Recipient Family and patient management
Requirements
- Location: Detroit
- Hours: 630AM-5PM, 4-10-hour shifts. No weekends or on-call requirements.

Qualifications
- Required: Bachelor's degree in nursing preferred.
- Organizational and interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families.
- Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.
- Analytical ability and motivation necessary to effectively plan, deliver, and implement plans of care to in acute and outpatient situations.
- Preferred: Experience in ICU or OR.
Skills
- Current RN license in the State of Michigan.
