Job Description
Performs clinical and socio-economic assessments on referred patients and navigates care between physicians and diagnostic services.
Connects patients and families with resources for education, support, and clinical needs throughout their course of treatment.
Works collaboratively with Patient Accounting and Coding to ensure compliance with patient charges and appropriate coding of procedures.
Provides effective follow-up with structural heart patients per mandatory registry guidelines and ensures records are in EMR for registry data abstraction.
Captures clinical data, compiles it, and assesses its appropriateness for presentation to the multi-specialty physician team.
Facilitates scheduling and authorization of procedures, both pre and post-procedure.
Engages with Marketing to enhance referrals and provide community education.
Uploads required data to appropriate vendors and maintains communication with device representatives.
Acts as a liaison between the patient and the multi-specialty physician team.
Knowledge, Skills, and Abilities
- Skilled in critical care nursing and operating room or catheterization lab experience
- Basic cardiovascular rhythm recognition and cardiopulmonary monitoring assessment
- Advanced knowledge of cardiovascular diagnostics and imaging techniques
- Excellent interpersonal and communication skills, written and verbal
- Ability to problem solve and multitask
- Highly organized in compiling clinical data / images for presentation to multi-specialty physician teams to accurately assess patients for advanced clinical procedures
- Diplomatic personality with strong drive to grow the program
- Proficiency in Microsoft PowerPoint, Excel and Word

- Associate's of Nursing
- Field Of Study in Nursing
Work Experience
- 1+ cardiac surgical critical care experience
- Minimum 5 years of experience in cardiac critical care as a Registered Nurse
