RN Clinical Team Lead - Rheumatology

About the role

The Sanctuary - Outpatient Center at 8960 Colonial Center Drive, Fort Myers, FL 33905 is seeking a RN Clinical Team Lead for a busy outpatient adult Rheumatology practice. This position requires 3 years of RN experience and outpatient experience is preferred.

Responsibilities

• Lead Clinical Quality Improvement
• Ancillary to this role, perform Medicare Annual Wellness Visits (direct patient care)
• Perform patient triage within the practice
• Manage the in-basket through MyChart
• Care gap closure by protocol
• Provide patient education and follow up on discharges

Qualifications

• Education: Graduate of an accredited school of nursing. Bachelors degree in nursing or healthcare field preferred.
• Experience: Minimum of Three (3) years of relevant RN experience in area of specialty required.
• Certification: Certification in appropriate specialty preferred. PALS/ACLS as appropriate for specialty within 90 days of employment. BLS required.
• Licenses: State of Florida RN license.
• Other: Project management and quality improvement experience preferred. Above average communication skills. Adaptable and participative with technology changes

Pay & Schedule

• Minimum to Midpoint Pay Rate: $33.37 - $45.05 / hour
• Schedule: Shift 1/7:00:00AM to 5:00:00PM
