About the role
Lee Physician Group is seeking an experienced RN to join their College Point practice to fill the role of RN Clinical Team Lead.
Responsibilities
- Provide monthly Clinical Team Education, along with additional training and retraining as needed.
- Manage in-basket clinical responsibilities, including prescription requests, result notes, and urgent symptom calls.
- Conduct protocol-based patient visits, including Flu/COVID care, urinary tract infection (UTI) visits, and hypertension (HTN) follow-up.
- Perform monthly CGM monitoring and reporting, including patient follow-up as needed.
- Maintain chronic condition reports and coordinate appropriate patient follow-up.
- Participate in Shared Governance initiatives and activities to support quality improvement and collaborative decision-making.
- Provide, oversee, and coordinate nursing care for patients within a complex department or highly procedural area.
- Deliver and document patient care using the nursing process of assessment, diagnosis, planning, implementation, and evaluation.
- Show critical thinking and problem-solving skills while serving as a clinician, teacher, researcher, team leader, and professional developer.
- Collaborate with other professionals to ensure effective and efficient patient care delivery and achieve desired outcomes.
- Enhance quality nursing care through excellent customer service and lean performance improvement techniques demonstrating positive outcomes in patient care.
Requirements
- Education: Graduate of an accredited school of nursing. Bachelors degree in nursing or healthcare field preferred.
- Experience: Minimum of Three (3) years of relevant RN experience in area of specialty required.
- Certification: Certification in appropriate specialty preferred. PALS/ACLS as appropriate for specialty within 90 days of employment.
- BLS required.
- Licenses: State of Florida RN license.
- Other: Project management and quality improvement experience preferred. Above average communication skills. Adaptable and participative with technology changes.
