About the Role
Manage and supervise in-home nursing care for medically fragile patients, ensuring high-quality service and compliance with policies. Lead clinical staff, perform patient assessments, coordinate care transitions, and validate nurse competencies. Collaborate with operational teams to optimize patient outcomes and staff development.
Responsibilities
- Oversee internal casework and nurse talent development
- Conduct Start of Care Assessments and Re-evaluations
- Review physician orders and update care plans
- Recruit, train, and validate clinical staff
- Enhance clinical expertise of field nurses
- Ensure regulatory compliance
- Coordinate with teams on patient care and discharge processes
- Travel to patient homes within the designated territory
Requirements
- Current RN license in the state of practice
- Minimum of 1 year nursing experience
- Valid driver’s license, reliable transportation, and ability to pass MVR check
- CPR certification (AHA or ARC)
- Willingness to supervise in-home nursing care for diverse patient ages
- Must meet health-related requirements, including vaccinations and testing, in accordance with jurisdictional mandates
