Job Summary

Provides patient care and multidisciplinary ambulatory team support. Drives evidence-based practice to improve patient outcomes as a dyad leadership partner with regional ambulatory clinical manager.

Responsibilities

• Educates patients and clinical staff on evidence-based practices.
• Consults with healthcare professionals to implement quality improvement initiatives.
• Identifies and addresses gaps in care, developing and evaluating interventions.
• Optimizes resource utilization to ensure cost-effective, high-quality health care.
• Trains and educates ambulatory clinical staff through informal and formal education programs.
• Supports staff onboarding, competency management, skills assessment, and development.
• Facilitates career-long opportunities for professional growth.
• Serves as a clinical expert, patient advocate, change agent, and role model.
• Guides clinical practice and performance by streamlining workflows.
• Aids in accreditation and regulatory educational requirements and documentation.

Qualifications

• Bachelor's degree in Nursing (BSN).
• Current RN license in good standing.
• Minimum 5 years of relevant clinical experience in ambulatory care setting.
• Experience in multidisciplinary team leadership and collaboration.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficiency in using electronic health records (EHR).
