RN Clinical Manager, Home Health

About the Role

The Registered Nurse Clinical Manager oversees clinical operations at a home health branch, ensuring quality patient care, staff management, documentation compliance, and regulatory adherence.

Responsibilities

• Review referrals and guide clinicians in care delivery.
• Support care planning, monitor patient outcomes, and ensure compliance with regulations.
• Participate in hiring, training, and performance evaluations of clinical staff.
• Coordinate communication among healthcare providers, patients, and families.
• Contribute to quality improvement initiatives and community engagement.
• Provide limited direct patient care when necessary.
• Support clinical operations and participate in quality assurance programs.
• Act as Branch Director in their absence.

Requirements

• Active RN license in the state of practice.
• Current CPR certification.
• Minimum of two years of home health nursing experience.
• At least two years of management or supervisory experience.

Preferred Skills

• Proficiency with OASIS documentation and HCHB (Homecare Homebase) software.
• Knowledge of CMS PDGM (Patient-Driven Groupings Model).
