RN CASE MANAGER - SPRING VALLEY (FULL-TIME)

About the Role

The Valley Health System has expanded into an integrated health network serving more than two million people in Southern Nevada. Starting with Valley Hospital Medical Center in 1979, the system now includes Centennial Hills Hospital Medical Center, Spring Valley Hospital Medical Center, Summerlin Hospital Medical Center, Henderson Hospital, Valley Health Specialty Hospital, and West Henderson Hospital.

This role focuses on achieving quality healthcare outcomes by establishing safe, individualized discharge plans and providing proficient, timely utilization management services to ensure maximum reimbursement for all patient visits. Success in this role requires proactive collaboration with patients, families, and the healthcare team.

Responsibilities

• Establish safe, individualized discharge plans for patients.

Requirements

• Graduate of an accredited school of nursing.
• Minimum of three years of experience in varied clinical settings.
• Two years of experience in Utilization Review, Utilization Management, or Case Management preferred.
• Knowledge of social and physical factors affecting functional status at discharge.
• Familiarity with community resources to meet post-discharge clinical and social needs.
• Computer proficiency, including word processing, spreadsheet, and data collection/management programs.
• Current license to practice as a Registered Nurse in the State of Nevada.
• Ability to demonstrate knowledge and skills necessary to provide care/service appropriate to the age of patients served.
