About the role
Provide in-home patient care, conduct assessments, oversee care plans, and educate patients and families on home health procedures.
Responsibilities
- Conduct in-person interviews with patients
- Manage and coordinate care with caregivers
- Communicate patient conditions with providers
- Promote preventative care to reduce readmissions
- Ensure continuity of care
- Deliver personalized patient care
- Collaborate with the healthcare team
- Promote patient education
- Support community health initiatives
Requirements
- Current RN license
- CPR/BLS certification
- Reliable transportation
- At least one year of home health or similar experience
Preferred Qualifications
- Experience in case management
- OASIS certification
- Trach/ventilator care experience
