RN Case Manager + Recertification Nurse - Home Health Agency (PartTime or PRN - Weekdays, daytime + evenings) | Shelby, OH (Mansfield) - NW OH

Responsibilities

• Manage patient care plans and ensure compliance with regulatory standards.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Provide ongoing assessment and reassessment of patients' health status.
• Ensure timely completion of recertification processes for assigned patients.
• Communicate effectively with patients, families, and other healthcare providers.
• Document patient interactions and care activities accurately and in a timely manner.

Requirements

• Bachelor’s degree in Nursing from an accredited program.
• Current RN license in the state of Ohio.
• Experience in home health nursing, including case management and recertification.
• Strong clinical judgment and problem-solving skills.
• Excellent interpersonal and communication skills.

Qualifications

• Knowledge of home health agency policies and procedures.
• Proficiency in electronic health record systems.
• Ability to work flexible hours, including weekends and evenings.

Skills

• Effective communication skills.
• Strong organizational and time management skills.
• Ability to prioritize tasks and manage multiple assignments simultaneously.
