RN Case Manager Recertification Nurse - Home Health Agency (Part-Time or PRN Weekdays, daytime evenings)
About the role
This is a Part-Time or PRN leadership position reporting directly to the NW OH Regional Program Administrator. For those pursuing career growth, a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing, Business Administrator, QA/Compliance leadership, and other positions will be supported.
Work consists of case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services. Additional responsibilities include new hire orientations, in-service and supervisory reviews, and input to influence training, education, care practices, outcomes, and overall success of clients, employees, and the business.
Role requires back-up and occasional skilled nursing visits including medication setups, dressing changes, and other services.
Responsibilities
- Perform client assessments and direct, plan, and initiate appropriate actions in home care situations
- Manage and supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides
- Document, communicate, and coordinate patient status and changes in condition to the care team
- Perform job duties while promoting personal safety and a safe environment for clients and coworkers
- Travel to surrounding counties as required
- Submit updated documentation of patient condition and ongoing care needs in compliance with state, federal, and agency guidelines
- Ensure successful communication with physicians, agency staff, service administrators, caregivers, clinicians, clients, and families
Requirements
- At least 1 year of experience in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership
- At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent); 2–5 years preferred
- Minimum of 1 year of nursing supervisory experience; home care experience preferred
- Graduate of an accredited School of Nursing
- Minimum of 1 year experience as a Registered Nurse with an active RN license in the State of Ohio
- CPR and First Aid Certifications (or obtainment prior to hire through agency or accredited third parties)
- Experience with OASIS and Non-OASIS assessments
- Strong background in nursing, patient care, and confidentiality
- Exceptional on-time and attendance record with consistent availability aligned to chosen schedule
- Familiarity with HIPAA regulations and medical terminology
- Ability to perform required physical demands
- Consent and ability to pass State & Federal background check (including fingerprints)
- 18 years or older with active Driver’s License/State ID and US Work Authorization
- Reliable transportation to all scheduled visits within stated travel radius and assigned clients (up to at least 2 hours required)
Desirable Qualifications
- Nurse supervision experience
- People management experience
- Peer/staff education and development
- Success or willingness to build relationships with client payors and case manager/entity relationships (e.g., AAA, DoDD County Board, VA) in NW Ohio
Schedule Type & Service Area
Weekday position (Monday–Friday) with primarily daytime availability (8 am–5 pm) and occasional evening availability. No weekends required.
Option 1 / Regional RNCM (PRN Only)
- Travel to clients in Findlay, Fostoria, Tiffin, and surrounding areas within a 30–60 minute radius
- At least 70% case acceptance rate within agreed-upon availability
- At least 1 case worked per 15 days (with exceptions), though more is desirable if needed
Option 2 / Road Warrior RNCM (PRN or Part-Time)
- Service area spans NW Ohio including Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and surrounding areas within a 2-hour radius
- Willingness to travel throughout this entire region required
- First priority of cases to maximize additional hours with the agency
- At least 50% case acceptance rate within agreed-upon availability
- At least 1 case worked per 15 days (with exceptions), though more is desirable if needed
- Qualified applicants may reside anywhere within the service area so long as travel requirements are fulfilled
Pay
Part-Time or PRN (+ Mileage): $36–$40 per hour based on employment and travel agreement and pre-determined availability of visits/services/recertifications per week. Overtime and quarterly bonus opportunities available. Paid weekly (Fridays) via direct deposit.
