RN Case Manager PRN

About the role
As an RN Hospice Case Manager, you will coordinate care for patients and families under the direction of the primary physician and interdisciplinary group (IDG). Your role involves identifying patient/family needs, developing and following an individualized plan of care, and utilizing monitoring, intervention, and teaching skills to enhance quality of life. You will ensure the highest level of personalized, sincere, and engaging service while maintaining patient confidentiality and addressing concerns promptly.
Responsibilities
- Support and carry out the organization’s mission, vision, and values.
- Coordinate care for the patient/family in collaboration with the primary physician and registered nurse.
- Identify patient/family needs and contribute to an individualized plan of care as part of the IDG.
- Maximize patient comfort and quality of life through monitoring, intervention, and teaching skills.
- Prioritize the comfort and care of patients and families at all times.
- Provide high-level, personalized, and engaging service to patients and families.
- Ensure patient and family satisfaction by addressing concerns promptly and effectively.
- Maintain confidentiality and respect in all aspects of patient/family care.
Requirements
- Registered Nurse currently licensed in the State of Florida.
- BSN preferred.
- Hospice certification preferred.
- Current Basic Life Support Certification.
- One year of experience in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing preferred.
- Understanding of Hospice Nursing principles, including case management, crisis intervention, communication, teaching, and thanatology skills.
- Basic computer skills and EMR software experience.
- Adequate and reliable transportation with valid Florida driver’s license and insurance.
Schedule
PRN, 8:30 AM – 5:00 PM with weekend availability.
