RN Case Manager PRN
Job Summary
This is a PRN Day Shift position. Hours are typically 8:00am - 4:30pm.
Qualifications
- Current RN license in the state of Louisiana or Registered Nurse diploma
- 2-3 years nursing experience in an acute care setting, case management experience preferred
- Certification in Case Management, Nursing, or Utilization Review preferred

About the Role
As a Case Manager, your role will be to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop care plans that reflect each patient’s unique needs. By partnering with physicians, nurses, and department leaders, you will help ensure safe transitions, clear communication, and consistent quality across the continuum of care.
Responsibilities
- Monitor the patient’s plan of care as developed by the physician to insure appropriate services are accomplished in a timely fashion
- Initiate the case management process prior to admission when possible, and extend it across the continuum of care until the patient no longer requires management
- Work independently with minimal supervision
- Report to the Director/Manager of Resource Management
Requirements
The Case Manager shall be responsible for coordinating the interdisciplinary team approach in assessing the need for discharge planning/case management. Also, is a resource to staff members on community services and resource management.
