RN Case Manager-Private Duty

About the role

Traditional Home Health Care, a CMS 4.5 Star-rated industry leader in Northeast Pennsylvania and winner of the HomeCare Elite 2018 and Readers’ Choice 2019 awards, is expanding its Pediatric & Adult Skilled Home Care Division. We are seeking a full-time, salaried Home Health (RN) Case Manager to join our family-owned, non-corporate team. This is a “skies the limit” opportunity where you dictate your own success.

Responsibilities

• Receive referrals, assess services required, and determine the Agency’s ability to meet the needs of pediatric and waiver patients.
• Conduct supervisory visits to observe clinical skills and ensure exemplary client care and compliance.
• Monitor documentation of pediatric and waiver patient clinical records for regulatory compliance in timeliness, accuracy, and completeness.
• Manage and supervise a team of RNs, LPNs, and Home Health Aides to deliver effective and quality home care services.

Requirements

• Current license as a Registered Nurse in Pennsylvania.
• Prior experience in scheduling, human resources, or customer service.
• Demonstrated organizational and time management skills.
• Excellent communication, interpersonal, and documentation skills.
• Strong critical thinking and ability to work independently.
• Ambition to grow and advance with company goals.
• Ability to multi-task in a fast-paced environment.
