About the Role
Join Hearth Hospice as a Full Time Hospice Registered Nurse Case Manager (RNCM) and make a lasting difference by providing exceptional end-of-life care. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As part of our interdisciplinary care team, you’ll ensure every patient receives compassionate, clinically excellent care grounded in dignity and connection.
Territory: Cleveland
Schedule: Monday–Friday, 8:00 AM to 4:30 PM

Responsibilities
- Respond to clinical referral information with expertise and compassion, addressing inquiries and requests to showcase our commitment to exceptional service.
- Partner with the Clinical Director to assess client eligibility and suitability for specialized hospice services.
- Provide direct care as an RN Case Manager, ensuring patient comfort and quality of life remain paramount.
- Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
- Educate and empower patients and families, encouraging their active participation in creating personalized care plans.
- Participate in call duties, demonstrating our commitment to being there when it matters most.
Requirements
- Current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year of experience as an RN in a medical-surgical or acute care setting (hospice experience a plus).
- Computer proficiency, including typing and experience with Electronic Medical Records (EMR).
- Current CPR certification (if required by state).
- Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay
$65,000 – $77,000 per year (average pay range). Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.
