RN - Case Manager

Responsibilities

• Provide direct patient care under the supervision of a physician.
• Assess patients’ health status and develop care plans.
• Monitor patients’ progress and adjust care plans as necessary.
• Collaborate with interdisciplinary teams to ensure coordinated care.
• Document patient care activities and communicate with healthcare providers.

Requirements

• RN, have a compact license or have a PA license.
• Length of assignment: 12 weeks.
• Shift / Hours per week: day shift, 8 hrs. a day with every other weekend required.
