RN Case Manager - Per Diem -Days - 8hr FPH

Job Summary

Captures and coordinates patient care to ensure quality and cost-effective outcomes. Collaborates with interdisciplinary teams both internally and externally. Engages in quality improvement initiatives relevant to patient care management.

Job Requirements

• Minimum Education: Bachelor’s degree in Nursing or related field preferred.
• Minimum Experience:
  • Newly graduated nurses, re-entry nurses, and nurses new to the U.S. healthcare system must complete the Emanate Health R.N. Residency Program within 6 months of employment.
  • Two years recent acute clinical experience required.
  • Experience with discharge planning, utilization management, and critical pathways is preferred.
• Skills:
  • Effective written, verbal, and time management skills.
  • Ability to work independently and build relationships within a team.
  • Knowledge of medical information systems.
  • Excellent customer service skills.
• Licenses:
  • California R.N. license required.
  • BLS certification required.
  • One hour of stroke education annually for those working at QVH.
