RN Case Manager - Per Diem - Days - 8hr QVH

Job Summary

Captures and coordinates the care of patients to ensure high-quality and cost-effective outcomes. Collaborates with both internal and external interdisciplinary teams. Engages in quality improvement initiatives relevant to patient care management.

Job Requirements

• Minimum Education: Bachelor’s degree in Nursing or related field preferred.
• Minimum Experience: Completion of Emanate Health R.N. Residency Program within 6 months of employment. Two years recent acute clinical experience required.
• Experience: Discharge planning, utilization management, and critical pathways are preferred.
• Skills: Effective written, verbal, and time management skills. Independent work capability and team-building abilities. Knowledge of medical information systems. Excellent customer service skills.
• Licenses: California R.N. license. Basic Life Support (BLS) certification required. One hour of stroke education annually for those working at QVH.

Qualifications
