RN Case Manager - Per Diem - Days - 8hr FPH

Job Summary

Captures and coordinates patient care to achieve quality and cost-effective outcomes. Collaborates with internal and external interdisciplinary teams. Engages in quality improvement initiatives relevant to patient care management.

Job Requirements

• Minimum Education: Bachelor’s degree in Nursing or related field preferred.
• Minimum Experience: Completion of Emanate Health R.N. Residency Program within 6 months of employment. Two years recent acute clinical experience required. Experience with discharge planning, utilization management, and critical pathways is preferred.
• Skills: Effective written, verbal, and time management skills; ability to work independently and build relationships within a team; knowledge of medical information systems; excellent customer service skills.
• Licenses: California R.N. license; Basic Life Support (BLS) certification required; annual 1 hour of stroke education required for those working at QVH.
• Availability: Available to work weekends.

Pay Range

$84.00 - $84.00 annually
