RN, Case Manager, Per Diem

Major Responsibilities

• Demonstrates a comprehensive knowledge of case management theory and practices.
• Demonstrates ability to effectively plan for patients' continuity of care needs in ambulatory care and emergency care settings.
• Clinical documentation in Medhost and Meditech
• Demonstrates understanding of the principles of Care Management
• Demonstrates accurate understanding of InterQual Criteria
• Demonstrates effective communication in a timely fashion with all involved departments concerning the appropriateness of bed status and insurance authorization
• Demonstrates effective communication with health plan case managers regarding the policies for the ECC patient.
• Demonstrates knowledge of general nursing theory and practices
• Demonstrates knowledge of discharge planning rules and regulations
• Informs director of any and all discharge planning incidents which are barriers to effective.

Position Qualifications

• All responsibilities are essential job functions.
• Licenses/Certifications/Educations Required: Current Massachusetts Nursing License Graduate of an accredited school of nursing, RN or BSN
• Experience/Skills Required: Experience in discharge planning and utilization review

Physical Demands And Environmental Conditions

• Moderate physical activity required including but not limited to walking.
• Must demonstrate emotional stability.
• May be required to work additional hours.
• Must demonstrate flexibility in relating to individuals of varying disabilities, and from different cultures, socio-economic and spiritual backgrounds
• This job requires that weight be lifted, or force be exerted.
• Good Physical Environment.
• May have to travel outside the hospital to attend workshops and conferences

• Responsible managers will review competencies with position incumbents.
