RN Case Manager - Per Diem

Seeking a registered nurse with a minimum of three years' clinical experience, including familiarity with social and physical factors affecting discharge and community resources. Responsibilities Include Establishing safe, individualized discharge plans to ensure quality healthcare outcomes.Providing timely utilization management services to maximize reimbursement.Collaborating proactively with patients, families, and healthcare teams to coordinate care. Qualifications And Skills Graduate of an accredited nursing school and current Nevada RN license.Knowledge of social, physical, and community resources relevant to discharge planning.Proficiency in computer applications such as word processing and data management.Experience in Utilization Review, Utilization Management, or Case Management preferred.Ability to provide age-appropriate care/services based on patient needs.
