Role Overview

The position involves providing safe, individualized discharge planning and proficient utilization management services to optimize healthcare outcomes and reimbursement. The role requires collaboration with patients, families, and healthcare teams to ensure timely and effective care transitions.

Responsibilities

• Establish safe discharge plans tailored to patient needs
• Collaborate with healthcare teams, patients, and families
• Coordinate utilization management to maximize reimbursement

Qualifications

• Graduate of an accredited nursing program
• Minimum three years' clinical experience, with two years in Utilization Review, Management, or Case Management preferred

Skills & Requirements

• Proficiency in computer applications including word processing and data management
• Current Nevada Registered Nurse license
• Ability to provide age-appropriate care and services
