RN Case Manager - Per Diem

Job Summary
Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care.
What you will need
- Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire
- Current RN license in the State of Michigan
- Preferred: Certification in Case Management or other related professional certification
- At least 2 years of experience in Case Management experience in an acute care hospital
- PREFERRED: 5+ years of Case Management experience

Requirements
- 16 hours/per month
- Some weekend requirements (approx 3-4/year)
- Holiday requirement will be one holiday per year
