About the role
Caregivers in Sanford Health's Case Management team play a crucial role in ensuring seamless transitions for patients from hospital to home or other care settings. They collaborate with interdisciplinary teams to address discharge needs and facilitate care coordination.
Responsibilities
- Collaborate with licensed practitioners and other inter-professional team members to provide care coordination, transitional care planning, facilitation and coordination of discharge needs.
- Promote patient wellness through evidence-based practice, improved care outcomes, efficient utilization of health services, and appropriate level of care for patients during hospitalization and on a continuum after discharge.
- Provide discharge-planning education and make referrals to inter-professional teams when necessary.
- Initiate links with post-acute external care resources.
- Anticipate and/or identify patient educational needs, discharge planning issues, and effectively collaborate to expedite early discharge planning.
- Act as an advocate for the patient, functioning in various care settings and across the continuum of care; achieving desired outcomes for both the clients and healthcare agencies.
- Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients.
- Collaborate with other inter-professional colleagues, including physicians, to plan, implement and evaluate care.
- Practice within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
Requirements
- Bachelor's degree in Nursing strongly preferred in Bemidji, Wisconsin, Michigan and Rapid City.
- Required in Fargo and Sioux Falls (Sanford USD Medical Center and Clinics).
- Required in Bismarck or enrolled and starting a Bachelor’s degree program within one year of hire with a completion date within three years upon date of hire into position.
- Required in Fargo Network and Sioux Falls Network, or employees hired after January, 2025, who do not possess a Bachelor’s degree in the required field, leadership may consider an educational plan with proven continuous action toward achieving a Bachelor’s degree in Nursing within five years of hire into position.
- Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
- Minimum of two years’ clinical experience preferred.
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
- Obtain and subsequently maintain required department specific competencies and certifications including Basic Life Support (BLS) within 60 days of hire.
- Certification in: Case Management (CCM), or holds a certification in their patient population specialty that they serve, is encouraged when eligible.
