As an RN Case Manager at MSK, you'll use your clinical judgment and nursing experience in a new way, helping patients and families navigate complex cancer care. Responsibilities
- Provide case management for a group of oncology patients
- Begin assessing discharge-planning needs when a patient is admitted
- Review each patient's progress and identify the services needed for a safe and timely transition to the next level of care
- Coordinate home care, equipment, services, and follow-up care
- Work as part of a multidisciplinary team to provide continuity throughout treatment and follow-up
