RN Case Manager, Hospice (Full-Time) - Bilingual Spanish & English

Candidate must be fluent in both Spanish and English. This is a Full-Time position, 8 am - 5 pm, Mon-Friday, with a minimum of 20-25 visits per week! Pay is $81 per/routine visit and $211 per/admission (includes non-taxable mileage reimbursement). Must be able to partake in the weekend and overnight on-call rotation! What You Will Do Have flexible hours and working close to home (reduce your commute!)Complete initial and on-going comprehensive assessments of the patient’s needsProvide service and /or treatments requiring substantial and specialized nursing skillsInitiate appropriate preventative and rehabilitative nursing proceduresCounsel, support and instruct the patient, family and caregiver in meeting nursing and related needs Who you are: Graduate of an approved school of professional nursing.Current, active license in the state of California Board of Nursing, Current CPR, health clearance, TB clearance.
