About the role
Assisted is seeking a full-time (open to per diem) RN Case Manager to join their Hospice Team in the Ventura branch. Spanish language proficiency is required.
Responsibilities
- Complete initial and ongoing comprehensive assessments of patients' needs
- Provide services and/or treatments requiring substantial and specialized nursing skills
- Initiate appropriate preventative and rehabilitative nursing procedures
- Counsel, support, and instruct patients, families, and caregivers in meeting nursing and related needs
Requirements
- Graduate of an approved school of professional nursing
- Current, active license in the state of California
- Current CPR certification
- Health clearance
- TB clearance
Qualifications
- Experience in hospice care preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
