About the role

We are seeking a dedicated and compassionate Hospice RN Case Manager to join our Optage Hospice team in the greater Milwaukee area. This is a meaningful opportunity to support the growth of a faith-centered hospice program, delivering exceptional, person-centered care to patients and families during life’s most important moments.

Responsibilities

• Lead and coordinate comprehensive patient care plans as part of the interdisciplinary hospice team
• Provide expert clinical assessment, symptom management, and end-of-life care
• Build trusted relationships with patients and families, offering guidance, education, and emotional support
• Collaborate with the Clinical Administrator and care team to ensure exceptional care delivery and outcomes
• Serve hospice clients in and around our Milwaukee area communities, currently focused in the Waukesha area

Qualifications

• Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
• BSN preferred.
• A minimum of three years of nursing experience preferred.
• Critical care/oncology, long-term care, or medical/surgical background with strong knowledge of pain and symptom management preferred.
• Hospice/Home Care experience preferred.
• Excellent interpersonal skills.
• Ability to work with dying patients and caregivers in the home, hospital, and other facility settings.
• Ability to work as a member of an Interdisciplinary Team.
• Experience with symptom management.
• Strong oral and written communications skills.
• Computer skills helpful.
• Reliable transportation, current driver’s license and current auto insurance.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
