About the role
The role of RN Case Manager at Hospice involves providing comprehensive care coordination for patients and families. This includes managing patient care plans, communicating with healthcare providers, and ensuring that all medical needs are met.
Responsibilities
- Develop and implement comprehensive care plans for patients
- Communicate effectively with healthcare providers and family members
- Ensure all medical needs are met and coordinate services as needed
- Monitor patient progress and adjust care plans as necessary
- Collaborate with interdisciplinary team to provide holistic care
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- At least 2 years of nursing experience
- Excellent communication and interpersonal skills
- Ability to work independently and manage multiple tasks
Qualifications
- Strong organizational and time management skills
- Experience with hospice care and end-of-life care
- Knowledge of Medicare/Medicaid regulations
Skills
- Strong clinical judgment and decision-making skills
- Effective problem-solving abilities
- Proficient in Microsoft Office Suite

Pay
The pay range for this position is $45,000 - $55,000 annually.
Schedule
The schedule for this role is Monday through Friday, 8:00 AM - 4:30 PM.
