About the role
Provide compassionate care to hospice patients and their families. Manage patient care plans, coordinate services, and ensure compliance with hospice standards.
Responsibilities
- Develop and implement comprehensive care plans
- Coordinate with interdisciplinary team members
- Communicate with patients and families about care options and progress
- Ensure all services are provided in accordance with hospice guidelines
Requirements
- Bachelor’s degree in nursing or related field
- Current RN license in good standing
- Experience in hospice or palliative care preferred
Qualifications
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office Suite
Skills
- Excellent organizational and time management skills
- Knowledge of hospice policies and procedures
- Ability to handle sensitive and confidential information
