RN Case Manager - Hospice

About the role

Join By Your Side Hospice Care LLC as a Hospice RN Case Manager, where you will provide compassionate care to patients and their families during life's most challenging moments. In this role, you will be an essential part of a dedicated team committed to enhancing the quality of life for our patients in South Eastern, Wisconsin.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Cook up and manage patient care, ensuring adherence to hospice policies and procedures.
• Provide direct nursing care, including symptom management and pain control.
• Educate patients and families on end-of-life care and available resources.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Document patient progress and maintain accurate medical records.
• Participate in on-call rotations to provide after-hours support.
• Advocate for patients' needs and preferences in care decisions.

Requirements

• Current RN license in the state of Wisconsin.
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care.
• Strong assessment, communication, and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• Compassionate demeanor with a focus on patient-centered care.
• Experience with electronic medical records (EMR) systems.
• Certification in hospice and palliative nursing (CHPN) is a plus.
• Willingness to travel within the service area as needed.

Compensation

$80,000.00 - $90,000.00 per year
