Location: West Madison, WI – Full-time 1.0, day shift (Monday-Friday, 8:30am-5:00pm). Includes on-call rotations (1 weekend per quarter), weekend shifts (every 8th weekend), and holiday rotations (2 per year). Sign-on bonus: $6,000.

About the Role

Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. As a Nurse Case Manager (RN Case Manager), you will deliver patient- and family-centered care, conducting assessments, creating and updating plans of care, providing interventions, and ensuring thorough communication and documentation.

Responsibilities

  • Patient and Family Assessments
    • Assess physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs as outlined in the plan of care (POC).
    • Collect and document data from patients, families, interdisciplinary team (IDT) members, and other healthcare professionals.
    • Identify, interpret, and document assessment findings, reporting significant changes or unusual incidents (e.g., abuse/neglect, suicidal intent, harmful coping mechanisms).
    • Collaborate with the IDT and primary care provider to create a Hospice Evaluation Summary (HES) for eligibility determination.
  • Patient Visits
    • Respond to patient and family needs, including changes in condition or requests for acute intervention.
    • Provide interventions, treatments, and pain management tailored to patient and family needs.
    • Evaluate and adjust pain management or other interventions for effectiveness.
    • Educate patients and families on pain/symptom management, end-of-life care, and related topics, evaluating and adjusting education as needed.
  • Plan of Care
    • Formulate, implement, and revise individualized POCs in collaboration with the IDT, patient, and family, including goals, pain management, interventions, education, and discharge plans.
    • Develop achievable outcome measures and establish care priorities.
    • Document interventions, outcomes, and changes in patient condition in the electronic health record (EHR).
    • Delegate visits to appropriate disciplines and oversee the work of Certified Nursing Assistants (CNAs) and Licensed Practical Nurses (LPNs).
  • Additional Duties
    • Participate in meetings to plan and evaluate patient/family services.
    • Precept new staff and students.

Requirements

  • ADN or BSN required.
  • Current, unrestricted Registered Nursing License in the state of employment.
  • One year of nursing experience preferred.
  • Valid driver’s license, auto insurance, and access to a reliable vehicle.
  • Must be at least 18 years of age.
  • Basic Microsoft Office skills and intermediate electronic health record (EHR) skills.
  • Ability to use proper body mechanics, safe patient handling, and standard precautions.
