RN Case Manager – Home Health

About the Role

Field-based home health RN Case Manager position in Waterboro, ME. This role offers autonomy, variety, and one-on-one patient time by driving to patients' homes to build relationships and make clinical decisions as the primary point of contact for each case.

Responsibilities

• Conduct in-home visits to assess patient needs and physically evaluate condition per physician orders.
• Travel within assigned territory to see patients on a scheduled basis.
• Coordinate with healthcare providers, patients, and families to facilitate quality care.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to patients and their families regarding care management.
• Document all case management activities accurately and timely in compliance with organizational policies.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

Requirements

• Current and valid RN license in the state of Maine.
• Experience in case management or related nursing role preferred.
• Strong assessment, organizational, and communication skills.
• Ability to work independently and collaboratively in a fast-paced environment.
• Proficiency with electronic health records and case management software is a plus.
• Valid driver's license and reliable transportation required.
• Willingness to travel to patient homes within the territory.
