RN Case Manager – Home Health

About the role

As an RN Case Manager, you will coordinate and manage patient care services, ensuring each patient receives high-quality, individualized care. You will collaborate with physicians, caregivers, and interdisciplinary healthcare teams to create and oversee effective care plans that improve patient outcomes and quality of life.

Responsibilities

• Complete comprehensive patient assessments and ongoing evaluations
• Develop, implement, and manage individualized care plans
• Coordinate home health services and community resources
• Monitor patient progress and adjust care plans as needed
• Collaborate with physicians, therapists, caregivers, and family members
• Educate patients and families on health management and wellness strategies
• Maintain accurate and timely clinical documentation
• Ensure compliance with all regulatory and agency standards

Requirements

• Current and active Registered Nurse (RN) license in Maine
• Minimum of 2 years of nursing experience
• Home health, care management, or case management experience preferred
• Strong clinical assessment and critical-thinking skills
• Excellent communication and organizational abilities
• Experience with electronic medical records (EMR) systems
• Reliable transportation and ability to travel throughout the Brunswick service area
