About the role
Travels to patients’ homes within an assigned service area to conduct assessments, develop and coordinate individualized care plans, provide skilled nursing services, educate patients and caregivers, monitor progress, collaborate with physicians and interdisciplinary teams, manage documentation, and ensure continuity of care.

Requirements
- Active ME or compact RN license
- BLS certification
- 2 years of recent home health case management experience
- Experience with Epic and Oasis systems
Preferred Qualifications
- Experience with IV therapy, tube feeds, VAC dressings, compression wraps, port access, ostomies, chest tubes, S/p catheters, and lab draws
Schedule
- 13-week assignment
- Shift: 8:00 AM – 5:00 PM
- Every 4th rotating weekend
- 3–4 days per month callback
