Position Overview
The Home Health Case Manager at St. Luke’s Health System in McCall, ID, plays a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge.

What You Can Expect In This Role
Full-time Schedule: Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts.
Opportunity to work primarily on weekends and cover occasional weekday shifts as well.
Manageable Caseloads: Daily visit requirements are below the state and national average.
Compensation: Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and its surrounding regions.
Additional Pay: On-call pay and applicable differential pay.
Bonuses: Sign-on bonus and relocation assistance available (where applicable).

Qualifications
Education: ASN or BSN in Nursing.
Experience: 2 years of RN experience; Home Health or Acute Care experience preferred.
Licenses/Certifications: Current RN license in the state of Idaho; Basic Life Support (BLS) certification.
Other Requirements: Valid driver’s license and proof of auto insurance.
