Case Manager RN, Home Health

Responsibilities
- Affords direct patient care and supervises the care provided by LPNs and HHAs.
- Works collaboratively with other home health disciplines to ensure comprehensive care for patients.
- Applies principles of learning to teach patients self-management techniques and methods to regain independence.
- Utilizes established departmental policies and home health regulatory requirements to provide care as prescribed by the patient’s physician.
- Provides on-call support as assigned for patients.
- May be required to perform tasks under emergency conditions, such as patient resuscitation in the home.
Requirements
- Current Washington State Registered Nurse license.
- Current BLS certification.
- Valid Washington driver’s license.
- Proof of insurance on automobile used for work.
- Driving record in compliance with OMC standards.
- Computer internet connectivity (provided by OMHH).
- Reliable automobile transportation for entire scheduled shift.
Qualifications
- Associate’s degree in Nursing (RN) required.
- Bachelor’s degree in Nursing (BSN) preferred.
- Minimum of one year recent experience in home health, acute care, or long-term care preferred.
Skills
- Knowledge of nursing principles and practices.
- Problem-solving skills.
- Independent decision-making ability.
- Ability to work in diverse and complex work situations.

Pay Range: $44.44 - $67.34
