Responsibilities
- Assesses, plans, implements, coordinates, and evaluates the nursing care of patients in their place of residence.
- Promotes continuity of care for patients on their assigned caseload.
- Provides direct patient care and supervises the care provided by LPNs and HHAs.
- Works in collaboration with all home health disciplines assigned to provide care for patients on caseload.
- Demonstrates a working knowledge of the principles of growth and development of patients 12 years of age and older.
- Applies principles of learning in teaching patient’s self-management and methods to regain independence.
- Utilizes established departmental policies and procedures and home health regulatory requirements in providing care to patients.
- Performs tasks under emergency conditions may require strenuous exertion (i.e., patient resuscitation in the home).
- Has contact with patients under a variety of circumstances and is subject to varying and unpredictable situations.
- Occasionally subjected to irregular hours.
- Maintains a keen awareness of personal safety.
- Provides on-call support as assigned for patients.
Requirements
- Current Washington State Registered Nurse license.
- Current BLS certification.
- Valid Washington driver’s license.
- Proof of insurance on automobile used for work.
- Driving record in compliance with OMC standards.
Qualifications
- Associate’s degree required.
- B.S.N. preferred.
- Minimum of one year recent experience preferred in home health, acute or long-term care.
Skills
- Working knowledge of nursing principles and practices.

Pay
FT 100% WORK SHIFT: Days
Pay Range: $44.44 - $67.34
Union: UFCW 3000
Shift Differentials/Premiums: Retention Incentive: $5000
Weekend & Holiday Shifts: Yes
On Call Shifts: Yes
Premiums: Weekend - $4.00/hour
Lead - $3.25/hour
Preceptor - $1.00/hour
Per Diem - 15% (on base plus steps, in lieu of benefits)
Certification - $2.00/hour
Weekend Coordinator Pay (chg) - 15% on regular pay
